Supports Planners (full time)
Requirements: Bachelor’s degree in sociology, social work, psychology, nursing or any related human services field and 2-3 years’ professional experience in case management. Must have current driver’s license and reliable vehicle to perform responsibilities required. Must successfully complete criminal background check prior to employment.
Responsibilities include case management services for individuals with disabilities and seniors to achieve their independent living goals through the planning of community-based supports funded by Medicaid. Courteous phone etiquette, excellent written and verbal communications skills, computer skills – literate in Microsoft Word. Preference will be given to experience in providing case management to persons receiving long-term care and/or home and community-based services, working knowledge of Medicaid regulations, and Medicaid waiver. Position will require travel in Mid Shore and Upper Shore of Eastern Shore of Maryland and to main office in Salisbury.
Hourly rate based on experience. Benefits package available including medical, dental, vision, retirement and life insurance.

Coordinator of Vocational Rehabilitative Services (full-time)

Bay Area Center for Independent Living is searching for a Coordinator of Vocational Rehabilitative Services to create business partnerships within our community and provide educational and job development skills to individuals who are seeking employment opportunities.
Education:
Human Services or Business-Related Degree and any combination of education, and experience that demonstrates knowledge of individuals with disabilities, public relations, case management and volunteer management.
Essential Job Requirements:
Educate consumers on employability skills, job performance skills and technical skills. Encourage consumers to pursue post-secondary education and vocational opportunities. Create work-based learning experiences for the youth who participate in our curricular and transition programs. Develop partnerships with local businesses to search for potential employment or volunteer opportunities. Work with participants to develop comprehensive work resumes, references and other information/materials needed for employment applications/placements, and work with participants to apply to and enroll in post-secondary education programs.

Health Wellness and Fitness Coach (full-time)
Requirements: Must have at least one year of experience in serving individuals with chronic health conditions or disabilities. Must have experience working virtually with consumers and be proficient in entering data into a software system. Must have experience in providing health and wellness activities. Having a certification in meditation, yoga, health wellness, or personal training is a plus. Must have a current driver’s license and be able to successfully complete a criminal background check prior to employment.
Responsibilities include working one-on-one or in a group setting with individuals who want to improve their health and well-being. Creating workshops and support groups that encourage peer-to-peer relationships. Working with the public-school systems, infant and toddler programs, health departments and senior service centers to improve the overall health and well-being of our Eastern Shore community. Hourly rate based on experience and education. Excellent benefits package including annual leave, sick leave, medical, dental, vision, retirement, and life insurance. This is a two-year grant position with the possibility of becoming permanent.
